Administrative Applications Change Request Form

A request for service may be submitted by paper or via email to this address. The following information may be used as a template for your request. Please note all REQUIRED fields indicated in CAPS.

The request should contain enough information to go through the procedure for technical evaluation and prioritization. Please note that it is very important to specify DUE DATE FOR USER TESTING and DUE DATE FOR PRODUCTION.

The Enterprise Information Systems (EIS) Directorate will evaluate the request at a technical level to determine:

  1. An initial estimate for the number of days to complete. (A detailed estimate can be completed when complete functional specifications are received by EIS from the customer.)
  2. Whether the request can or cannot be scheduled by the TEST due date requested.
  3. Any technical comments by EIS staff.

The request will be entered into an electronic system for tracking and a Change Control number will be assigned to the request. A Service Representative will then inform the Customer of the Change Control number and the information above.
